A 17 year old patient presented with a Class III malocclusion. The maxillary anteriors were proclined and protrusive and the mandibular anteriors were retroclined and retrusive. There was a decreased overjet and overbite present with space shortage in both arches and an impacted upper left canine tooth 23.

The treatment plan consisted of a modified Rapid palatal expander incorporating a buccal extension arm. The extension arm was designed to facilitate the disimpaction of the impacted canine.

The tooth 23 was surgically exposed and a polycarbonate crown was bonded to allow for open eruption. Once the tooth had erupted further, the crown was removed and the tooth was bonded.

Follow up photos were taken 8 months post deband showing a stable occlusion.

Orthodontic treatment of Cleft patients can be both challenging and personally rewarding. Treatment times are usually extended and oral hygiene is often difficult to maintain.

A 12 year old male patient presented with a unilateral cleft lip and palate. He had a Class III malocclusion. There was severe space shortage in both arches with impacted teeth 15,25 and 35. An increased velopharyngeal orifice and arch asymmetry were also present.

The treatment plan consisted of fixed banding of both arches followed by extractions of teeth 15,25,35 and 45. The prognosis of the tooth 12 remained very poor. due to the lack of alveolar bone surrounding this tooth, this tooth was eventually extracted.

A pontic tooth was placed during treatment for the 12 area to ensure that the patient could smile with confidence. Total treatment time was 38 months.

At the end of the orthodontic treatment the patient was referred to his dentist for an interim bonded Maryland bridge to close the edentulous space.

A 14-year-old patient presented with a Class II Division II malocclusion characterized by retroclined and retrusive anterior teeth. The patient exhibited an increased overjet and overbite, along with impacted canine teeth 13 and 23.

The treatment plan involved the extraction of the retained deciduous canines (tooth 53 and 63). Fixed banding of both arches was completed to optimally align the dentition and to create space for the impacted teeth.

The tooth 23 erupted spontaneously and was aligned into the arch. To facilitate the eruption of the impacted tooth 13, surgical exposure was carried out by a maxillo facial surgeon , Dr.M A Bobat. A polycarbonate temporary crown was bonded to the impacted tooth 13 after exposure to aid in its guided eruption into the arch (open exposure)

The polycarbonate crown was removed after a few months, the tooth 13 was bonded and brought into the arch. The case was completed uneventfully.

This is a great example of teamwork between different dental specialisties to achieve our objectives.

Follow-up photos taken 8 months post-deband demonstrated a stable and well-maintained result.

A 20 year old male patient presented with a Class III skeletal relationship with severe crowding and the presence of gingival recession.

After discussing all possible treatment options with the patient and his parents, a non surgical treatment plan was chosen.

The treatment plan consisted of fixed banding of both arches to together with extractions of the 15 and 25 and the lower 34 and 44.

Follow-up photos were taken 7 months post treatment displaying a stable occlusion. .

An 13 year old female patient presented with a Class II division II malocclusion. Increased overjet and overbite and space shortage in both arches. A deep bite, microdontia and arch asymmetry was observed.

The treatment plan consisted of fixed banding of both arches to optimally align the dentition.

We used digital design to fabricate bite turbos that were specific for this patient. This customization aided in disoccluding the bite during the initial leveling phase of treatment.

Photos were taken 18 months post deband showing stable results.
